We have patients with complex and chronic medical conditions that require them to have a team of MD's. This can be much more than 2 outside providers which is the current limit.  The normal number for our patients is at least 3 and may be as high as 5-8. I need to associate their MD's with a patient. 

Suggest that once associated with a patient, there is a simple list of them by last name, first name, specialty, & phone. If you have additional screen real estate, add practice name. If one needs more info they can click on a provider and it pops their full contact info page.
