Need option not to use coversheet when faxing. When sending info to disability offices, pharmacies, or insurance they often request that a cover sheet not be used and their document (with their barcode) be on top. 

Also, please provide the ability to design our own fax cover sheet. We use it as a marketing tool and have the typical fields, but use the rest of the space to explain our practice.
