I just set up a new patient appointment template and created a questionnaire covering HPI, Past Medical History, Family History, Social History, and ROS. I added individual widgets for each component to the questionnaire and linked it to the note template I’m using for visits.

I tested it with a sample patient and can see the results in the provider portal and download them as a PDF. However, when I try to reconcile them with the note, I get an error saying "no data to reconcile." I’ve spent a lot of time building this and would really appreciate any help. Thank you!
