I am looking to find a way to simplify the consent and privacy document acknowledgments. Within the questionnaire framework it seems I would be able to post most of the aspects of my documents and then ask questions along the way to elicit consent. This would lack a signature. Is this advisable or is the only way really to have the patients print forms, sign them and email them back before their appointment? Or ask them to read the forms and be prepared to sign using the kiosk function on an ipad or something equivalent? I'd like to make the process as quick as possible for patients.
