I have a pain scale of 1-10 with each number able to click for each area of complaint. My problem is if I have multiple areas of complaint and I need to use 2 customized forms, I cant click a number for each.

For example:

If a neck pain is "6" I can click that but if I have to add shoulder pain "4" then the neck pain "6" goes away. 

Not sure how to correct this.
